Since Diablo 2, the Cow Level has been one of the greatest mysteries and a popular Easter egg in the series. In Diablo 4, it has finally been found… more or less. Because a corresponding achievement is now attainable, but is this really the end?
As already suspected, Lord of Hatred enables the final steps to the secret Cow Level. There were already hints beforehand, including shirts from the developers, a cow mug on the desk, and strange items that are now in the game.
The Cow Level is probably the biggest secret in the history of Diablo and is actually just a joke that has somehow taken a life of its own. Since the release of the new expansion, dozens of fans have been searching for the level with a whole Discord server – successfully, as it seems.
In the “Not finding a Cow Level” Discord, users have finally found a Cow Level. Or more precisely: a Cow Island. After about a week of searching in the new DLC, there are now first guides on how to access the level.
Rolling dice, fishing, and back to the start
A word of warning: the road to Cow Island is long. This is (still) not a real step-by-step guide, but merely an overview of what you need to do to enter the Cow Island.
You need some items from earlier steps of the search for Cow Island, some of which can already be done since the base game:
You must merge these two items in the Horadric Cube from Lord of Hatred with a third item: Neyrelle’s Hand. This requires several steps, which YouTuber Sliver of time describes in detail in his video:
- You need to click on a shrine somewhere on the volcano to trigger the events.
- Then you must return to Kyovashad and interact with the shrine where you at the very beginning of the original campaign scratched something onto an amulet.
- Next, you need to accomplish some things in Skovos, like collecting ballista bolts and reaching hidden places, collecting and wearing a white item set, and following a raven that… flies a very silly path.
- In between, you need to keep returning to Kyovashad and back to the shrine.
- Finally, you should return to where Neyrelle lost her hand at the very beginning of the game and start fishing. This will lead you to a hidden area where you will find Neyrelle’s Hand.
If you put the hand together with the bardiche and the stamina potion into the cube, you will receive a Cow Skull. Upon use, this teaches you the “Moo!” emote and you receive the achievement: “There is no Cow Level” as well as a torn diary page.
With this diary page, you can board a boat in Skovos that takes you to a small island where various things can be found:
- a bucket of milk
- the map of the area of Scosglen, which looks like a cow’s head
- a cellar
- a horse skeleton with rainbow “blood” underneath, apparently a nod to the unicorn-level Whimsyshire from Diablo 3

Just an empty island – Is there more to come?
The achievement suggests that the puzzle with the new steps is finally solved. However, the “meaning” behind the island is still a bit lacking. Previously, the Cow Level was an ideal place to farm certain items, but in Diablo 4, it currently serves no apparent purpose.
Additionally, there are still some special items that we found during testing that currently have no use. It is quite possible that the Cow Level has not been completely deciphered yet – after all, it is just an island at the moment. Perhaps the Cow Level is also the demons that we slaughtered along the way.
The probability that there is still something that can be done with the Horadric Cube is relatively high. After all, there are secret recipes – like the one with Neyrelle’s Hand – of which surely not all have been discovered yet.
