The survival MMO Fallout 76 features a pronounced role-playing aspect. You can customize your character through various features, including mutations.
What are mutations? With mutations, you can change your character attributes in Fallout 76. Mutations offer you benefits, which you must pay for with other drawbacks.

List of 19 mutations in English
What mutations are there? Mutations bring a lot of possibilities in Fallout 76. You can change various things with them. The full list in English can be found here:
- Eagle Eye: +4 Perception, -4 Strength
- Adrenaline Reaction: Increases weapon damage when health is low, decreases your maximum health by 50
- Charged: Chance to electrify enemies when they attack in melee, but decreases maximum health by 50
- Marsupial (as seen in the video): +20 carry weight, increased jump height, -4 Intelligence
- Chameleon: As long as you are not carrying a weapon and not moving, you are invisible
- Egghead: +6 Intelligence, -3 Strength, -3 Endurance
- Empath: Team members take 25% less damage, player takes 25% more damage
- Grounded: +100 Energy Resistance, -20% Energy Damage
- Flesh Eater: Meat provides double benefits, vegetables provide no benefits
- Healing Factor: +300% health regeneration, -55% effect of chems
- Pack Mentality: +2 Intelligence in a group, -2 Intelligence when alone
- Unstable Isotope: Chance to radiate enemies when they attack in melee
- Claws: Unarmed attacks deal 25% more damage and cause bleeding, -4 Agility
- Muscle Relaxation: +25% melee damage, increased chance to cripple opponents, -50% accuracy with firearms
- Herbivore: Vegetables provide double benefits, meat provides no benefits
- Speed Demon: +20 movement speed, faster reloading, 50% hungrier and thirstier while moving
- Scaly Skin: +50 damage resist, +50 damage reduction, -25 Action Points
- Plague Walker: Poison aura that deals more damage to enemies the more diseases you have
- Bird Bones: 4+ Intelligence, reduced falling speed, -4 Strength
Most mutations have an impact on your character’s attributes. They work in Fallout 76 like in its predecessors using the S.P.E.C.I.A.L. system. Mutations thus have a similar mechanism to perks or traits from the previous games.
The battle royale mode Nuclear Winter also offers special, unique mutations.
How to get and keep a mutation
If you want to mutate, there are two ways: the slow, uncertain way and the expensive one. If you want to keep a mutation, you can easily do so through a perk.
Mutating through radiation
This is how you mutate through radiation: In Fallout 76, your character mutates by exposing it to high radiation. You can stay in contaminated areas, consume irradiated food, or be attacked by radioactive monsters like ghouls. Radiation weapons from other players, like the Radium Rifle, can also cause you to mutate.

Whether you then mutate is left to chance. Whenever you become irradiated, you have the chance to mutate. However, you have no influence over which mutation you get.
Buying mutations with a serum
Mutating with a serum: If you want to obtain a mutation deliberately, you can do so with a serum. There are several ways to obtain a serum:
- Make it yourself with a corresponding recipe
- Buy it from players who can produce the serum
- Buy the serum directly
The serums and their recipes can be found at the MODUS science terminal in the science wing of the Whitespring bunker.

Keeping mutations
How do I keep mutations? If you have found a mutation you like, you can even keep it permanently.
You only need to equip the Luck perk “Enhanced Genes”. This will prevent you from removing mutations in any way at rank 2. Information on all perks in Fallout 76 can be found in our guide.
Removing mutations
This is how you get rid of mutations: If you involuntarily mutate, you can reverse the mutations by taking RadAway. However, you only have a chance of having the mutation removed.
However, with the perk “Enhanced Genes”, you can already prevent mutating at level 1.
