Step 4: Enter the cow island
With the three items you have collected, go to the Horadric Cube. Place the stamina potion, the rusty bardiche, and Neyrelle’s hand inside and click “Transmute.” You will receive the “Trophy of the Believers.”
Just a reminder: this is how you obtain the Horadric Cube.
When you use the trophy, you will receive the achievement: “There is no cow level,” a title, the emote “Moo!” as well as the “Torn Page.” Head to the eastern coast of Philios with the page, a bit north of the Cetos peninsula.
On the beach there, you will find a rowboat. Interact with the boat, and you will see a short cutscene with a cryptic text about “tears of milk” and other things.
On the island, you can interact with various items, including:
- a horse skeleton lying in a rainbow puddle
- some notes
- a milk bucket
- as well as a portal to another cellar
If you fish on the island, you also have (allegedly) a chance to catch cows. Whether this is true, we currently cannot confirm.
Congratulations, you have found the cow level! Or maybe not. Because there is no cow level. Just an island. Without cows.
Is there more after the cow island?
It is currently uncertain whether the steps explained here truly represent the end of the puzzle. The fact that you receive an achievement suggests this. However, it contradicts the fact that at the end of the journey… there is nothing. No further reward, no item, no secret level to power-farm like in Diablo 2.
Moreover, at the end of each previous step in Vanilla Diablo 4 and Vessel of Hatred, there were secret cellars full of cows to be killed. There is a cellar on the island too – yet (still) no cows.
Additionally, there is still the puzzle with the coins and the wish well, which has now received a new class to experiment with thanks to the Warlock. However, how and whether this puzzle relates to the cow level remains unknown.
Lord of Hatred still has further secret items related to cows, such as a rusty bell that can be fished, or the crown of the cow king as a piece of equipment. It is likely that these – or other – items must be combined to unlock another step of the puzzle.
