Step 4: Enter the Cow Island
With the three items you have collected, you now go to the Horadrim Cube. Place the Stamina Potion, the Rusty Bardiche, and Neyrelle’s Hand inside and click on “Transmute.” You will receive the “Trophy of the Believers.”
As a reminder: this is how you obtain the Horadrim Cube.
When you use the trophy, you will receive the achievement: “There is no Cow Level,” a title, the emote “Moo!,” and the “Torn Page.” Take the page to the eastern coast of Philios, a bit north of the Cetos Peninsula.

On the beach there, you will find a rowboat. Interact with the boat, and you will see a short cutscene with a cryptic text about “tears from milk” and other things.
On the island, you can interact with different items, including:
- a horse skeleton lying in a rainbow puddle
- some notes
- a bucket of milk
- as well as a portal to another cellar
If you fish on the island, you also have the chance to catch cows.
Step 5: The Cow Level and the Cow King
Once you have reached the island, there is one last step you need to take to open the actual Cow Level. The good news: this time it goes really fast. The bad news: it only works on one day of the week.
Return on a Tuesday to Kyovashad and select the last remaining sin at the shrine: Greed. Then head back to the start of the puzzle in Skovos, to the Diary of the Exiled, and interact with it. You will now receive a new text in which the author says he wants to see this strange realm for himself.
If you now go back to the Cow Island, you can kill the twelve cows below in the cellar. Finish them off, and a golden portal to the Cow Level will open:
The realm, which suspiciously resembles Temis, is full of cows with bardiches, as known from older versions of the Cow Level. Somewhere deeper in the level, you will then find the Cow King, a special boss that you can only encounter there. If you defeat the king, you will receive a mythical item, the Cow King’s Crown, with strange effects depending on the current day of the week:
- Moo-day: +1 damage against two-legged enemies (when leg x2 = damage)
- Tuesday: 10% reduced chance of having bad luck against Pi times thumb distance away enemies. This distance corresponds to 3 to 18 hooves when one assumes this afternoon is a parallel to the equator.
- Wednesday: Chance to reduce enemy cooldown (no).
- Thursday: After picking up a legendary item, you deal increased damage to non-elite stunned enemies between 3:00 AM and 4:00 AM for 2.5 seconds by 3%.
- Friday: +10.26% Moo-bility damage while moving
- Saturday: +1.3% chance to ignore cow troll loss if you have already suffered cow troll loss for more than 10 weeks from the hell flood
- Sunday: +51 life upon death (+maybe).
Is there anything else going on now?
For the Cow Level, that is rather unlikely. The Cow King is traditionally the boss of the level, and with him, the mystery is essentially solved. However, it remains to be seen how often the Cow Level can really be accessed – is it only on Tuesdays? Or anytime after the first opening? Even just once per character? This information is still pending.
However, there is still the puzzle with the coins and the wishing well, which has now received a new class to experiment with thanks to the Warlock. So there are certainly no shortages of secrets in Diablo 4, even though the biggest one has now been solved.
