Intellect Devourer

Intellect devourers are cruel, small creatures that devour the brains of their victims and take over their bodies – hence their name. They possess high intelligence and typically live in remote or underground areas, such as the Underdark from which they originate.
With their tentacle-like head and beak-like mouth, they not only look completely absurd but are also particularly dangerous to magical or especially intelligent beings, whose abilities they can use for their own nefarious purposes – or those of their mind flayer, who created them.
When an intellect devourer has possessed the body of a victim, the original consciousness of that victim is erased and lost forever. As a result, their new hosts often also serve as disguises. So you never know if you might be facing an intellect devourer.
