4. Colin Farrell as Billy Callahan

Turk and J.D. encounter Billy, who allegedly is the brother of a patient who ended up in the hospital after a bar fight. He emphasizes in the episode how important it is to truly live life, and he quickly becomes a favorite among the staff at Sacred Heart.
However, it turns out that he is not actually the brother of the patient, as Billy actually fought him. After J.D. and Turk call the police, the situation becomes clear, and Billy turns into a small role model for the two doctors.
Colin Farrell is a great actor, and he shows this even in the short episode of Scrubs. He is the true star of the episode, not only having great chemistry with everyone but also carrying a message.
