On Reddit, an equipment item with the highest item level was reported on June 23, which has an item power of 870 after the upgrade.

What kind of item is this? The item is the two-handed sword “Infernal Edge,” which has an item power of 845 and is presented by u/5altwaterCowboy in his post. He writes there that he had nothing better to do at level 100 and had farmed “Almunn.”
“Almunn” is a unique monster in Diablo 4 that drops a special item upon being killed. There are several of these in the open world, as we have listed here.

The properties of the two-handed sword are also quite good. The sword has not only a high damage per second of 3.022 but also a double boost for critical hit damage (+52.5%, +63%), an increase of 93% for damage to injured opponents, and +72 to all stats.

How does he get the high item power? The item has an original item power of 845, which is already quite high. 5altwaterCowboy went to the blacksmith with his new two-handed sword and upgraded the sword 5 times.
Every upgrade increases the item power by 5 points as well as the base value, such as armor or damage per second. Thus, he reached an item power of 870.
According to Icy Veins, it is speculated that drops from special elite enemies can reach a value of up to 850. However, there have been no reports beyond this (via icy-veins.com).
When is an upgrade worth it?
Is an upgrade always worth it? Upgrading items is worth it if you have materials in abundance and want to take the boosts, as well as if you are sure that you want to keep the items. We recommend this when you are close to the endgame and from an item power of about 750.
