We will see Mephisto in Vessel of Hatred:
Mephisto is the oldest evil and is dangerous not only to humans and angels but even to his allies. The Lord of Hatred is considered the most intelligent and cunning evil. It is said that his mere presence is enough to corrupt a land, and even fragments of him are strong enough to taint everything they touch. In Diablo 4, Mephisto initially made a secret appearance.
As the “eldest brother,” Mephisto plays Diablo and Baal against each other, making him the demon most likely to win in a war for the hells. As the world is trapped in an eternal conflict, with constant hatred toward the other side, he can always benefit as the Lord of Hatred.
What truly makes him the most powerful evil are his long-term planning and influence on the world. He is the father of Lilith and Lucion. Without Lilith, there would be no Sanctuary, and without Lucion, the evils never would have gained a foothold in the world. Through him, the story of Diablo was made possible.

The Prime Evil: Tathamet
At the absolute top of the ranking, someone else must stand: Tathamet. Few of you probably know this being. Tathamet is the second creature that ever existed, right after his brother Anu.
Through the battle and death of the two brothers, the High Heavens and the Burning Hells were created. From Tathamet’s seven heads, the seven evils were ultimately born.
In addition to the evils, there are numerous other adversaries in Diablo who would certainly deserve a place on the list of the worst foes – not least the Butcher. However, one character stands out particularly, because he has committed perhaps the greatest crime against humanity: An angel killed 90 % of the humans in Diablo 3, thus exciting fans for Diablo 4.
