What was the original plan? Bill Lawrence, the creator of Scrubs, originally planned only one season for Scrubs, as he did not know if the show would be well-received. Thus, the season finale was also meant to be the series finale. A clever thought, as even today shows are regularly canceled.
As Screen Rant reports, the big twist in the finale was supposed to be that J.D. was only imagining his greatest adversary, the janitor. You can still sense that plan within the first episodes, as the janitor only interacts with J.D.
Surprisingly, the show was popular and NBC extended the series. The actor playing the janitor then asked if he could also interact with the other actors. This happened, and the janitor became an important figure who had stories not just with J.D. He also formed a band or got married. One thing remained until the end: he was J.D.’s nemesis.
