With Vessel of Hatred, Diablo 4 receives its first expansion and thus a new antagonist: Mephisto, the Lord of Hatred and Lilith’s father. Many of you surely remember the Prime Evil from earlier, but do you also know how important he actually is for the story?
If you play Diablo 4, you will encounter Mephisto quite early in the game. He saves you from a spell, but appears there in the form of a wolf and gives you some cryptic messages along your way.
The further you progress in the story, the more you will deal with the evil. This makes sense: Mephisto is the father of Lilith, the main antagonist in Diablo 4. By the end of the campaign, the Lord of Hatred is even more or less freed.
This is where Vessel of Hatred comes in. The expansion follows the story in which Neyrelle has stolen Mephisto’s soulstone to escape to Kurast.
However, those who have just started with Diablo 4 may not really know this strange skull-headed figure. MeinMMO explains who Mephisto is and why he is more important for the lore of Diablo than any other character.
You can find all the info on Vessel of Hatred in our summary here. In the intro, you can see Mephisto’s evil power:
Mephisto: The First of the Three Prime Evils
Mephisto originated, like the other six evils, from one of the heads of the Prime Demon Tathamet. However, the Lord of Hatred is the first to emerge from the heads of the three Prime Evils (Baal, Diablo, Mephisto). He is the oldest evil. Here are a few details about Mephisto:
- His true name is Dul’Mephistos.
- He is the “Lord of Hatred,” feeding primarily on hatred—which is abundantly present in the Eternal Conflict between angels and demons.
- Mephisto is considered the most intelligent of all evils. He not only has considerable combat strength, but primarily plans and plays his opponents against each other.
- For some, Mephisto is seen as the true ruler of Hell. If there were to be one who could lead it, it would be the Lord of Hatred.
To stoke hatred, Mephisto not only creates discord between already hostile factions but also occasionally plays his allies against each other. Where the hatred ultimately comes from is largely irrelevant to him.
He is also regarded as an excellent general. Because many, even powerful demons, bow to his commands and follow his advice, he is seen by some as the leader of Hell.
Supposedly, Mephisto’s influence is so strong that fragments of his power are sufficient to corrupt everything that comes into contact with it permanently. In the game, you first encounter him in Diablo 2. There he can be found in Act 3.

Mephisto is More Important for Diablo than Diablo Himself
Mephisto has a much greater significance for the story of Diablo—more even than the namesake Diablo himself. Diablo doesn’t even appear in Diablo 4.
However, Mephisto is important not so much as an actual character, but mainly as the father of Lilith and Lucion. Without these two, there would firstly be no world of Diablo, and secondly, we would never have reason to fight against the demons:
Lilith stole the Worldstone and, together with her “beloved,” the angel Inarius, created Sanctuary and the Nephalem, from whom humans later emerged.
Lucion led the Triune, a religion that aimed to strengthen the demons’ influence in Sanctuary under the pretense of benevolence.
What is the Worldstone?
The artifact, also known as the “Heart of Creation,” grants something akin to omnipotence. With a thought, realities can be created and worlds destroyed. The Eternal Conflict, that is, the battle between the High Heavens and the Burning Hells, revolves solely around the Worldstone.
Only through his two children does it even come to the point that we must fight against demons (and partially angels) in 5 video games today.
For more background on the lore, Blizzard tells in the video series The Book of Lorath

Without Mephisto, We Wouldn’t Fight Anyone Today
Sanctuary was originally hidden from demons like angels until a few human wizards summoned demons in their hubris. This is how Mephisto became aware of the world.
To gain the world and thus the Worldstone itself, Mephisto pursued a plan. The plan is the Triune, a religion led by his son Lucion.
The reason for the secrecy is that the evils did not want the High Heavens to become aware of Sanctuary. Because the powerful Nephalem and humans were to become his weapon. The Triune knows three “gods”:
- Dialon, the Spirit of Determination – the guise of Diablo, the Lord of Terror
- Bala, the Spirit of Creation – Actually Baal, the Lord of Destruction
- Mefis, the Spirit of Love – Actually Mephisto, the Lord of Hatred
The further a believer delves into the seemingly benevolent religion, the closer they come to its “true” teachings. The Triune slowly corrupts its members, turning them into demon worshippers.
Inarius, who is still roaming in Sanctuary, recognized the plan and, as a counterpoint, founded the Church of Light—also disguised as a prophet who is essentially considered a god by the believers. However, the two religions long existed in a kind of arms race, what is now known as the “Sin War.”
Mephisto’s Children Are the Reason We Can Play Diablo Today
Lilith, Mephisto’s daughter, is ultimately responsible for the Eternal Conflict reaching Sanctuary. After her banishment, she returned to the world she created and there birthed the second generation of Nephalem—starting with a farmer named Uldyssian.
Here you can find everything you need to know about Lilith.
Although Lilith’s plan did not quite work out, Uldyssian rises to power and kills Lucion. In the ensuing dispute over the leadership of the Triune, Lilith appears disguised as Lucion and takes over, only to intentionally plunge the Triune into complete chaos.
Through the story, Uldyssian becomes powerful enough that the High Heavens also take notice of Sanctuary, and the Eternal Conflict eventually reaches the world of humans.
Ultimately, it may not be Mephisto himself, but at least his offspring that allows us to play Diablo today while being quite powerful. It’s even conceivable that this has been his plan all along. We will only find out what happens with him in Vessel of Hatred. You can find more background here: Lilith is the reason we are still alive – Why are we fighting her?
