In Diablo 4 there are unique monsters that give you special items. And the item power of these items is also special.
What kind of monsters are they? These unique monsters are spread throughout the open world of Diablo 4 and drop a special item upon being killed.
Players in the deepest endgame have discovered that the special items from these monsters are significantly above the usual power limit. Normally, it ends at 820. However, these items can exceed 840, possibly even up to 850.
Where you can find the monsters and what the catch is, you can find out here.
Diablo 4: Finding Unique Monsters
Where can you find the monsters? You can check out an interactive map and/or pay attention to certain signals on your mini-map.
Here is a map with the unique monsters, the link is in the image description:
And this is what the signals on the map look like when you are near such a monster:

An English list of all enemies and what values the items can have can be found at Fextralife (via fextralife.com). Click on the enemy and then in the article about the enemy on the item.
To get to the monster “Qiniel” in Kehjistan, you must initiate a conversation with Prinstess Qin several times. Then there is a chance that she will transform.
Was there a catch? On the one hand, the items are class-specific and it may be that the strong item drop does not help you.
More importantly: These items always have the same secondary values when dropped and always have only 3 values. Yellow items, however, often come with 4 values.
However, you can also have values that normally don’t exist on these equipment pieces. There could be interesting combinations for some builds.
Are the items worth it? An item with high item power can definitely be worth it, even if the secondary values don’t fit.
The Diablo expert wudijo has already looked at some items and believes that many value combinations are really poor and hardly fit into a build (via youtube.com). For hunters, he sees potential, for the other classes currently rather less.
What is still strong about these items, besides the item power: They only cost a handful of gold. This means you can often re-roll a value with the occultist without immediately needing to apply for housing assistance. If you have enough materials, it remains relatively cheap.
It is definitely worth trying to trade the high item power with 3 values for an item with 4 values.
